I hereby acknowledge that I am participating in strength training and competition with the full understanding these activities may involve a significant risk of bodily injury, including death or damage to property of myself and others.

I understand that it is not possible to list specifically each and every risk. I hereby expressly accept sole liability and responsibility for all risks of injury or death associated with the use and results of competition engaging with Nexus Performance. I acknowledge that this is elective participation and I am undertaking this of my own volition. I agree that in exchange for and in consideration of Nexus Performance permitting me to participate in the competition, I am hereby assuming all the risks associated with the competition; including but not limited to the use of facilities and equipment.

I hereby agree to release and hold harmless Nexus Performance, its trustees, directors, subcontractors, officers, agents and employees, from any and all liability, actions, including, but not limited to, claims for negligence, recklessness or any other form of action for which a release may be legally given (including legal fees and costs) which may arise by or in connection with my participation in any activities related to the competition.

This disclaimer can be pleaded as a bar to all actions against Nexus Performance, this is not limited to later life injuries that may have been suffered while under guidance and protocols of training and nutrition with Nexus Performance.

By acknowledging this disclaimer you assume liability for injuries that may follow, you also accept that the guidance provided is not subject to a regulated field of experience and that information provided is as a result of knowledge and experience developed and expanded on by each coach over a period of time. The jurisdiction of effect is to be Queensland, Australia. If any portion of this release from liability shall be deemed by the relevant jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions shall have no effect. *

 I declare myself to be physically sound and suffering from no medical condition, impairment, disease, infirmity or any other illness that would prevent my participation in these activities or use of equipment or machinery.

I voluntarily and knowingly accept the risk of injury by my participation in this event entails. I certify that I am 18 years of age or older, or that I am the parent/guardian of the participant competing and that I have read this document and fully understand it.
